I hereby authorize the strata corporation to withdraw from my bank account the following special levy or other charge on the payment due date, or, if that date is in the past, the next possible deduction date following the submission of this form.

**Authorization may only be submitted by a registered owner. If the strata lot is owned by a corporation, the submission must come from a director of the corporation.

**Email must match email address submitted to the strata corporation by a registered owner.

The full levy or other charge amount(s) will be deducted on the first Electronic Funds Transfer (EFT) date following the due date(s).
